Methodology

We surveyed more than 2,000 physicians in the region to find out where they would send a family member in each of 76 specialties, and most doctors agreed on who is the best: The survey revealed clear winners.

The names of the physicians surveyed were chosen randomly by computer from lists of practitioners in Baltimore City and the five surrounding counties.

Looking over our shoulders were three physician advisers, below, who were not eligible to be on the list. To be a "Top Doc," physicians selected by their peers needed to be accepting new patients and needed to respond to our requests for background information.

Q: How did you choose this specialty?
A: There were two things I liked about surgical oncology. One was dealing with patients who have a very complicated problem that we can really help, where there is a lot of research and development going on. Also, because it allowed me to do very complex surgical procedures that involve many different organs.

Q: What are the biggest changes in cancer treatment over the last few years?
A: One thing that has changed things tremendously is what we call personalized cancer care, which is not only dealing with the patient on a person-to-person basis, but being able to identify the areas in a gene analysis that can help us determine whether the patient will do well, whether they will respond to therapy, and most importantly, what therapy we could use to help them.

Q: Tell me about this procedure you've become known for, which was recently mentioned in The Washington Post?
A: We have patients that come to us from all over the United States and all over the world for a procedure that few institutions do now, people with peritoneal carcinomatosis, which is when tumors spread all over the abdomen, from the colon, the stomach, the appendix, the ovary. We go in and, in 12, 14 hours of surgery, remove all the tumors and use heated chemotherapy. We have people who were told to go home and die who are now alive 15 years later.

Q: You grew up in Colombia and have also worked in Glasgow and traveled extensively for your work. How does U.S. health care compare with other places?
A: The medical care in the Unites States is excellent, probably the best in the world. Unfortunately, not everybody gets access to it. I think there's a lot of waste in medicine at all levels, caused by the malpractice that has not been touched [in the health-carereform debate]. If there's going to be a major change in health care, there has to be some kind of tort reform to decrease the cost of things. There are many areas in Maryland that do not have obstetricians because an obstetrician has to pay over $100,000 a year on insurance to practice. It's crazy.

Q: Why did you choose to focus on high-risk pregnancies?
A: I wanted to combine medicine with surgery and I wanted to take care of women along the path of life, from young to old. Then, as I got involved in that, I realized, from a purely intellectual perspective, that a lot of the fun and the challenge of obstetrics is how fast the field changes. From a medical and personal perspective, many women who get pregnant get ill. And in order to cope with that illness, you need to learn a little bit more about how medical disorders such as diabetes, hypertension, and other underlying or preexisting diseases impact pregnancy, so that drove me toward high-risk obstetrics.

Q: What are the biggest challenges in cases like these?
A: Medications change so rapidly that it's rare you have a chance to test every medication on pregnant women. Many times now, we test things by default because you're not going to have large trials anymore saying this medication is specifically going to do this thing to this woman. I'm talking about really high-risk pregnancies, when you have underlying disorders like renal failure or diabetes, where some of the drugs that come along, you don't have 100 percent conclusive evidence that they do no harm. So you try to balance the risk to the fetus with the benefit to the mom.

Q: Do you think doctors today are too quick to do Cesarean sections when a natural birth is possible?
A: Nature designed it that we have natural birth, so I am a big proponent of delivering vaginally. There are many things that are driving the Cesarean section wheel. One is this concept of choice, selective Cesarean section. You have people coming in, saying 'All things being equal, doctor, I'd like to have a Cesarean birth instead of a vaginal birth.' The other thing is sort of an expectation between physicians, patients, and the public in general, that they want the best outcome under all circumstances and a Cesarean birth might seem more predictable. I still think that under all circumstances a vaginal birth, or attempted vaginal birth, is better. You can always go to the Cesarean birth as the next logical step, if for some reason you have an indication to do that.

Q: Why did you choose to specialize in cardiology?
A: When I was in my internship and residency, it was either cardiology or neurology. Cardiology stood out because you didn't need to do much testing. It was basically talking to the patient and doing a thorough exam, and that's why I like it.

Q: And do you specialize in women's heart health?
A: I do see a lot of women, but I also see a lot of men who want to see a female physician.

Q: Do you have a sense of why some men prefer a female physician?
A: I do. I'll tell you a story. My grandfather was about 95 when he died and he had his first heart attack when he was 85—this was 20 years ago—and he was admitted to Union Memorial and the doctor on duty was Miriam Cohen. My grandfather came from the old school and there was no way that he was going to see a female, and the first time he went to see her in the office, she had him strip down to his boxers and he was intimidated and appalled. But he grew to like her because she was so thorough and kind and didn't miss a trick and he was sold. The men that I see, I think they like me because I'm thorough and I listen and I let them talk and we spend a fair amount of time learning about each other. I don't think it's me, I think a lot of female physicians have that bent to them, they like to listen. I guess it's sort of a generalization, but a lot of the male physicians that are in my practice, it's in-and-out, in-and-out. "Chest pain? No? Ok, good. Bye."

Q: Do you ever face men who are reluctant to see a female doctor?
A: Now and then. But it doesn't take long to win them over. And now and then, there's a woman from the old school, who believes that doctors were men, and who I have to coerce into coming back.

Q: What have been the biggest changes in heart care from when you began?
A: The biggest is the advances in intervention for heart attack, with the stenting and being much more aggressive in that way. The other one, that's even more satisfying for me, is the treatment for heart failure. People with heart failure used to die within five years of diagnosis. Now the medications and the devices we have are so good that these people are living quality lives for many years.

Q: How did you end up in this particular specialty?
A: I first got into pediatric sleep medicine by my interest in Sudden Infant Death Syndrome (SIDS). I was head of the clinical SIDS unit at University Hospital. As time went on, people would call me to see if they could use the lab at night for older children who had different nighttime problems like snoring or who were sleepy during the daytime. I began to get interested in different sleep disorders, so I set up the first pediatric sleep lab at University Hospital. Then when I left downtown and came out to Baltimore County in 1999, I did more and more sleep medicine.

Q: You were recently quoted in The Sun talking about sleep apnea in children. We don't generally think of snoring and sleep apnea as childhood disorders.
A: Around the age of 6 or 7, children develop large tonsils and large adenoids, and the combination tends to block off their airway so that they're not only snoring, but, in some cases, they actually have sleep apnea and sometimes it can be severe. And that impacts their daytime lives, in terms of not doing well in school or being hyperactive without really having ADHD. So, yeah, kids do snore. In fact, a few years ago, the American Academy of Pediatrics recommended that all pediatricians ask about snoring during sleep, and if the parent says, "yes, my child snores more than three nights a week," that's habitual snoring and you want to think more about that child.

Q: Does childhood obesity have anything to do with it?
A: Yes, it does. We used to talk more about snoring and sleep apnea being more of a problem with the younger kids because that's when the tonsils and the adenoids get big, but now that we're having such an increase in teenage obesity, we're seeing teenagers who have sleep apnea.

Q: What other changes have you seen in the field?
A: As we see the high schools starting their classes earlier in the day than the middle schools, this means that teenagers are having to get up even earlier. And that's a problem because usually, if you remember back to your own teenage years, we tend to not fall asleep as easily once we become teenagers. Some of my patients can't fall asleep until 2 or 3 in the morning and then they have to get up for school at 6:30. These are kids who have what we call a circadian disorder. The circadian clock in their brain gets reset, usually at puberty, and they can no longer fall asleep easily at 9:30 p.m.

Q: Has there been much pressure to change the schedule?
A: There's starting to be, there have been some school districts around the country that have recognized the physiological facts and have had the younger kids start school earlier and the teenagers start later, but there's just a lot of inertia in our society.
